The phrase "test tube baby" is a non-medical layman's term used long ago because the embryo was formed outside the body (In-Vitro) in a laboratory, rather than inside the woman’s fallopian tube (In-Vivo). As technology modernized, this outdated term gradually diminished. Today, the precise scientific term IVF is the global standard.

What is IVF in Fertility?

IVF is an assisted reproductive technology in which a female egg is mixed with male sperm outside the body in a petri dish. Once fertilized, it is transferred securely to the woman’s uterus to establish a pregnancy.

Controlled Ovarian Hyperstimulation (COH)

The initial cycle begins with ovarian stimulation. We utilize precise protocols—ranging from letrozole to exogenous gonadotropins (FSH and LH). GnRH is often used to prevent premature ovulation. Follicular development is strictly monitored via ultrasound. Once lead follicles reach the appropriate size, final maturation is triggered by an HCG injection.

Oocyte (Egg) Retrieval

Exactly 34-36 hours after HCG administration, mature eggs are retrieved. Performed in a surgical suite under IV sedation, follicular fluids are scanned by embryologists via ultrasound-guided aspiration to locate all available mature eggs, which are then cultured in a special incubator.

Insemination & Fertilization

For normal sperm, 50k-100k motile sperm are transferred directly to the culture dish (Standard Insemination). For abnormal sperm parameters, we use the ICSI technique: a single spermatozoa is selected and directly injected into the egg cytoplasm using a fine glass microneedle, bypassing the zona pellucida.

Embryo Culture

The fertilized eggs (zygotes) are cultured in special formulation media that helps them grow. They are carefully monitored over 2 to 5 days by our embryologists to assess their development rate and cellular symmetry.

Embryo Transfer

After selecting the highest quality embryo, it is loaded into a fine catheter and transferred under transabdominal ultrasound guidance into the uterus. The number of embryos transferred depends on embryo stage, quality, and maternal age.

Embryo Quality & Transfer Timing

Assessing the quality of the embryo is critically important. The embryos are pictured and evaluated carefully by the embryologist early in the morning based on development rate and appearance.

Cleavage Stage (Day 3)

When embryos develop approximately 4-8 cells on Day 3, they are in the cleavage stage. During this stage, embryos are analyzed for cell symmetry and fragmentation.

Fragmentation occurs when cells divide unevenly. In our labs, these qualities are rigorously graded to select the most transferable embryos.

Blastocyst Stage (Day 5)

5th-day embryos are called blastocysts. They have increased in size and look similar to a ball of cells containing fluids inside.

The most important evaluation here is expansion; the more expanded, the better the quality. Blastocyst transfers have a much higher implantation potential, allowing us to transfer fewer embryos and avoid multiple pregnancies.
